Indications/Uses
Severe hypoglycaemic reactions, which may occur in the management of insulin-treated childn & adults w/ DM. Motility inhibition in GIT exams in adults.
Dosage/Direction for Use
Severe hypoglycaemia Adult & childn >25 kg or >8 yr Administer 1 mg by SC or IM. Childn <25 kg or <8 yr Administer 0.5 mg by SC or IM. Inhibition of GI motility Adult Diagnostic dose to relax the stomach, duodenal bulb, duodenum & small bowel: Administer 0.2-0.5 mg by IV or 1 mg by IM. Diagnostic dose to relax the colon: Administer 0.5-0.75 mg by IV or 1-2 mg by IM.
Contraindications
Special Precautions
Immediately administer after reconstitution. Must not be administered as an IV infusion. Therapeutic indication: When patient has responded to treatment, oral carbohydrates should be given to restore liver glycogen to prevent relapse of hypoglycaemia. Little or no effect when patient has been fasting for a prolonged period, or is suffering from adrenal insufficiency, chronic hypoglycaemia or alcohol-induced hypoglycaemia. Cannot assist in transference of carbohydrate from much larger stores of glycogen present in the skeletal muscle. Patient should not drive or operate machinery after a severe hypoglycaemic event until stabilised. Latex-sensitive individuals. Diagnostic indication: May experience discomfort (reports of nausea, hypoglycaemia, & BP changes), in particular if fasting. After the end of a diagnostic procedure, oral carbohydrates should be given to patients who have been fasting, if compatible w/ the diagnostic procedure applied. Observe caution in patients w/ insulinoma; in patients w/ glucagonoma; when used as an adjunct in endoscopic or radiographic procedures in diabetic patients or in elderly patients w/ known cardiac disease. Driving a vehicle & operating machinery should be avoided until patient has had a meal w/ oral carbohydrates. Safety & efficacy for inhibition of GI motility in childn & adolescents have not been established.
Adverse Reactions
Therapeutic indication: Nausea. Diagnostic indication: Hypoglycaemia (uncommon).
Drug Interactions
Antagonistic reaction w/ insulin. May lose the ability to raise blood glucose or paradoxically may even produce hypoglycaemia w/ indomethacin. May increase the anticoagulant effect of warfarin. Temporary increase in pulse & BP w/ β-blockers.
